Telehealth Informed Consent

# Telehealth Informed Consent You are about to receive medical care via telehealth — the use of secure video, phone, audio, or messaging to consult with a licensed clinician. ## What telehealth is and isn't Telehealth is not the same as an in-person visit. Some conditions require an in-person exam. Your clinician will let you know if your situation needs in-person care. ## Risks and benefits - **Benefits:** convenience, faster access, continuity of care. - **Risks:** technical failures may interrupt the visit; not all conditions can be diagnosed remotely; rare risks of privacy breaches despite our security measures. ## Your rights - You may withdraw consent at any time. - You may request that no recording is made. - Your clinician will use the same standard of care they would in person. ## Emergencies Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room. ## Acknowledgment By proceeding, you acknowledge that you have read and understand this consent, your questions have been answered, and you agree to receive care via telehealth.
