This agreement explains what you pay, when you are charged, and what happens if you cancel. Please read it before you check out.
UpLiftRx is a cash-pay service. UpLiftRx and the UpLiftRx-affiliated medical practices do not accept insurance, are not in-network with any commercial health plan, and are not enrolled as participating providers in Medicare, Medicaid, TRICARE, or any other federal or state healthcare program. We do not submit claims to any payer on your behalf.
By proceeding you acknowledge and agree that:
The price of your program, the billing interval, and exactly what is included are shown to you at checkout before you pay. Those checkout terms control for your plan.
A typical membership includes:
A typical membership does not include:
The amount you pay may include fees charged by the medical practice for your Provider's services, by the pharmacy for your medication, and by a laboratory for testing, together with the fee for use of the UpLiftRx platform and our administrative and support services. Amounts owed to clinicians, pharmacies, and laboratories are collected on their behalf and passed through to them. UpLiftRx does not retain or divert fees owed to a clinician, pharmacy, or laboratory. The balance is the fee for the technology, customer support, coordination, and administrative services we provide.
Programs are billed on an automatically renewing basis to the payment method you provide, at the price and interval shown at checkout, until you cancel. You will be charged even if you do not use every service your membership includes. To support continuity of treatment, a renewal charge or shipment may process up to two days early around holidays or for operational reasons.
You authorize us and our payment processors to charge your payment method for all amounts due, to retry a declined charge, and to update your stored card details when your card issuer provides them.
Prices may change. We will give you notice of any price change before it applies to your next renewal, and you may cancel instead of accepting it. If a price is displayed in error, we may correct it and adjust or cancel the affected order, refunding anything already charged.
Cancel at any time from Account → Plan & billing or by emailing support@upliftrx.co. Cancel at least two (2) days before your renewal date to avoid the next charge. Cancellation takes effect at the end of the period you have already paid for; you keep access until then. We do not pro-rate partial periods. After cancellation you will not receive further shipments or clinical services under that program, and your associated prescription may be cancelled with the pharmacy.
Talk with your Provider before stopping treatment — stopping some medications abruptly can affect your health.
Refunds are governed by our Refund & Shipping Policy. In summary: you are refunded in full if your Provider determines you are not a candidate for treatment or if we cannot serve your state; membership periods already begun are not pro-rated; medication that has shipped cannot be returned or refunded, because law prohibits pharmacies from accepting returned prescription medication; and laboratory services already performed are not refundable. Approved refunds go back to your original payment method, typically within 5–10 business days.
If a payment fails, we may pause your program until it is resolved, and shipments and clinical services may be suspended. If you believe a charge is wrong, contact support@upliftrx.co first — we correct billing errors promptly. Filing a chargeback before contacting us may result in your account being suspended while the dispute is investigated. You are responsible for amounts properly owed, and for reasonable costs of collection where permitted by law.
